PARTICIPANT WAIVER AND RELEASE OF LIABILITY U.S. Football Academy Effective Date: March 27, 2026

Please read this waiver carefully before registering for any U.S. Football Academy program.

Assumption of Risk I understand and acknowledge that participation in football training activities, camps, and clinics organized by U.S. Football Academy involves physical exertion, contact, and inherent risks including but not limited to sprains, strains, fractures, concussions, and other injuries. I voluntarily assume all risks associated with participation.

Release of Liability I, on behalf of myself and my minor athlete, hereby release, waive, and discharge U.S. Football Academy LLC, Coach DB Malone, their staff, volunteers, and representatives from any and all claims, damages, or liability arising from participation in any training session, camp, clinic, or program, whether caused by negligence or otherwise.

Medical Authorization I confirm that my athlete is physically fit to participate in football training activities. In the event of a medical emergency, I authorize U.S. Football Academy staff to seek emergency medical treatment on behalf of my athlete. I accept full financial responsibility for any medical treatment required.

Photo and Video Release I grant U.S. Football Academy permission to photograph and record video of my athlete during training sessions and camps for use in marketing, social media, and promotional materials without compensation.

Refund Policy All registration fees are non-refundable unless a program is cancelled by U.S. Football Academy. In the event of cancellation by U.S. Football Academy a full refund will be issued within 7 business days.

Acknowledgment By registering for any U.S. Football Academy program I confirm that I have read, understood, and agreed to this waiver in its entirety.
